When you study MBBS abroad, there is one rule that completely decides your future as a doctor in India:
Parents and students, please note that this rule applies only to students who go abroad for MBBS after 2021.
So, do not compare yourself with those who went abroad before 2021 — for them, these rules do not apply.
This is one of the major rules passed by the National Medical Commission in the official 2021 Gazette.
And apart from this, there are other rules you must follow as well.
But this rule is the most important that you must know, because each country has its own licensing procedure, and you should choose a country based on how easy or difficult the licensing procedure will be.
This requirement comes directly from the official NMC Gazette 2021 – Clause 4(b).
This is not optional.
This is not just a guideline.
This is a strict and mandatory rule for all students who study MBBS abroad after 2021.
According to the NMC Gazette:

In simple words:
This means the foreign medical graduate must be recognised in the same way as a local doctor is recognised.
In other words, the graduate must be able to practice medicine independently, without guidance or supervision from a senior doctor.
Here’s why this is important:
In some countries, like Georgia, MBBS graduates are allowed to work as junior doctors immediately after graduation.
A junior doctor cannot sign prescriptions or provide medicines independently. They work under the supervision of a senior doctor, like an intern.
According to NMC 2021, a foreign medical graduate must become an independent practitioner to be considered a licensed doctor.
Being a junior doctor is not enough, because only an independent doctor is legally allowed to practice and make decisions without supervision.
To protect students.
Some countries provide MBBS degrees but do not allow those students to get a license there.
This means the country itself does not treat them as doctors.

the degree has real value
students are legally allowed to treat patients in the country of study
Only when a student gets this foreign license, they become eligible to take FMGE/NExT and later apply for an Indian medical license.

Russia
Licensing Exam Language: The licensing exam in Russia is conducted in Russian, which creates a significant challenge for Indian students who are not proficient in the language.
Risk: Failure to pass the exam means students cannot practice medicine in Russia, and the degree becomes invalid for Indian practice under NMC guidelines. This can cause delays in fulfilling NMC’s 10-year completion requirement.
Georgia
Licensing Hurdle: According to Georgian medical law (Article 5), MBBS graduates can only work as junior doctors under supervision, not as independent practitioners. This contradicts NMC’s requirement for independent practice eligibility in the country of study.
Implications for NMC: Graduates from Georgia may face rejection of their license to practice in India.
Extended Duration: To qualify for independent practice in Georgia, students must pursue a Postgraduate degree, adding 2-3 years to the standard 6-year MBBS program. This results in a total duration of 9+ years, making it nearly impossible to meet NMC’s 10-year limit for degree completion, internship, licensing, and NExT exam.

In the context of selecting the right university for medical studies abroad, the term “NMC-recognized” refers to universities meeting the standards set by the National Medical Commission (NMC) of India. While the Medical Council of India (MCI) previously maintained a list of recognized medical universities for MBBS abroad, the responsibility has now shifted to the NMC.

Despite the absence of a specific list, the NMC’s Foreign Medical Graduate Licentiate Regulations 2021, introduced in November 2021, define the criteria for recognizing degrees from foreign medical universities. Any university that adheres to the regulations outlined in the FMGL Regulations 2021 will be acknowledged by the NMC, enabling graduates to appear for the National Exit Test (NEXT) in India.

Before finalizing plans to study medicine abroad, it is crucial to thoroughly review and ensure that the chosen university complies with the FMGL Regulations 2021. (1.) Foreign Medical Degree with minimum duration of 54 months. (2) Must have undergone an internship of twelve months in addition to the course, in the same foreign institution. (3) Received a foreign medical degree with entire course in English. (4) FMGs need to be registered with the respective professional regulatory body or otherwise, competent to grant license to practise medicine in their respective jurisdiction of the country in which the medical degree is awarded. (5) The entire course, training and internship or clerkship shall be done outside India in the same foreign medical institution throughout the course of study and no part of medical training and internship shall be done in India or in any country other than country from where the primary medical qualification is obtained. (6) The total duration of the foreign medical graduation course shall be completed within ten years from the date of joining the course.
